Responsibility
1. Appointments - I
am pleased to serve you and will strive to meet your needs. We will
reserve time in our schedule to accommodate your needs. It is important
that you keep your appointed time. Should an emergency arise to preempt
that, a 24 hour notice is expected so the time will be available for
someone else in need.
2.
Telephone Calls - Only a reasonable number of phone calls can be returned
per day. Our staff is trained to assist you so that we can care for our
scheduled patients with a minimum of interruptions. Emergency calls will
be handled immediately.
2. Prescriptions
- We treat illnesses and prescribe medications only after a through
history and physical examination. For your safety and in keeping with
acceptable medical practices, you must keep your follow-up appointments.
2. Medication
Refills-Refills
should be made during business hours. Please give 48 hours notice.
3. Acute Illness &
Injuries -Require an
office visit for proper evaluation and treatment. |
